Protracted Withdrawal Syndrome (PAWS): Withdrawal symptoms may last weeks to more than 12 months, including anxiety, cognitive impairment, depression, insomnia, formication (crawling skin sensation), weakness, shaking, muscle twitching, paresthesia, and tinnitus .
Acute withdrawal symptoms (life-threatening) include: catatonia, convulsions, delirium tremens, hallucinations, mania, psychosis, seizures, and suicidality .
Post-acute withdrawal syndrome is estimated to affect 10% to 25% of long-term benzodiazepine users and may persist for months or years after discontinuation . Medical detoxification with gradual tapering is recommended .
Suicidal Thoughts and Behavior
Like other antiepileptic drugs, clonazepam may cause suicidal thoughts or actions in about 1 in 500 people . Pooled analyses of clinical trials showed AED-treated patients had approximately twice the risk of suicidal thinking compared to placebo .

Contraindications

Pregnancy and Breastfeeding
Pregnancy: Late-pregnancy use may cause neonatal sedation (breathing problems, sluggishness, low muscle tone) and withdrawal symptoms (jitteriness, irritability, restlessness, shaking, excessive crying, feeding problems) .

Breastfeeding: Clonazepam passes into breast milk and may cause sleepiness, feeding problems, and decreased weight gain .
Drug Interactions
Clonazepam is metabolized by CYP3A4 :
CYP3A4 Inhibitors (bicalutamide, erythromycin, high-dose fluconazole, haloperidol) may increase clonazepam effects
CYP3A4 Inducers (carbamazepine, high-dose dexamethasone, phenobarbital) may decrease clonazepam effects
Phenytoin interaction: Unpredictable effects on each other's concentrations; monitor phenytoin levels
